TfS professional referrals to TfS ONLY.
If you work for a Sutton service and would like to refer someone who lives, works, studies or cares for someone in the London Borough of Sutton, please submit a professional referral here:
Our referral forms will be looked at by the TfS SPA (Single Point of Access), and someone will contact your client within two working days to see how best to support them.
Please be sure to list the TfS partners you feel the referrals might need to go to, or if you are not sure, complete the form as clearly as possible, listing all concerns and issues, we will then be sure to triage the case and pass across to the relevant TfS partners at the right time in your client's TfS journey.
