TfS Self-Referrals
If you live, work, study or care for someone in the London Borough of Sutton and you would like to refer yourself to TfS, please submit a self-referral here, we may be able to help you, or at the very least, point you in the right direction:
Our services are free and we won't pass your information on to anyone without your say so.
Our referral forms will be looked at by the TfS SPA (Single Point of Access), and someone will contact you within two working days to see how best to support you. If you know who you need to speak to, it is better to go directly to them so as not to add time to your referral, and if you would like to speak to one of our TfS partners, then please click directly on their contact buttons below.
We will respond to your enquiry as quickly as we can, please note that our telephone lines are open from 9am – 5pm, Monday – Friday, excluding bank holidays.
